Be sure to complete the form by including:
First & Last Name
Address, City, State & Zipcode
Phone Number
Contact Name
Description of Parade Entry (Float, Car, Animal, Bicycle etc...)
Note: Your description information will be read from the announcer's stand so please provide all applicable information on the form.
Liability Waiver: The Camden Point Freedom Festival Committee, or any other involved partiers will not be held responsible for any accidents, vandalism, fire, theft, or loss incurred during the Camden Point Freedom Festival. A signed copy of this waiver must be provided to the parade coordinator prior to the provide.
